Some people as they gain weight, tend to collect fat in the upper part of the neck. Even with old age there can be a collection of fat and loosening of skin in the upper part of neck behind the chin. This shows up as what is commonly referred to as a “Double chin.” The skin may also start sagging leading to the "Turkey Neck" deformity. There are two components to this deformity. It could be an isolated collection of fat alone as seen in younger individuals or could be because of lax musculature & skin also, along with the fat deposition, in the older age group.There is also a lack of the definition of the lower jaw wherein the neck and the face appear continuous. In younger individuals, a liposuction alone should be enough but older patients may require a tightening of the muscle along with a liposuction, to give good results. If only liposuction is done no significant scarring is seen but, in the case where muscle tightening has to be done, there is a 2 inch scar just behind the chin in the upper neck.
Post operatively, the patient is required to wear a chin strap (a pressure garment). This will help the skin shrink and give the necessary tightness to the skin to give a nice angle between the jaw and the neck.
No significant problems are expected except maybe the scar and the skin remaining loose if proper pressure garments are not used.
Neck Lift Proper:
Sometimes, there may be an excess of neck skin along with the fat. In mild cases a small incision behind the chin is sufficient to take care of the excess. Sometimes in minor cases where the skin excess is more in the sides than anteriorly, threads can be used to pull the skin back. In moderate cases the incision maybe behind the ears to pull the skin and the muscle back and fix it to the tight layer behind the ear and out to remove the excess skin .
In more severe cases (Bull neck Deformity) a neck crease scar from one side to the other, is needed to remove the excess skin which can be combined with liposuction of the neck and also excision of the fat and tightening of the muscle to pull back the neck and reproduce the proper angle between the jaw and the neck.
